Werneth Medical practice is a member of the Oldham West PCN (previously known as Milltown Alliance PCN)

Oldham West Primary Care Network (PCN) is made up of 8 GP practices across Chadderton and Werneth, who are working collaboratively with the aim of improving patient services, community well-being and through joint working will create efficiencies that can be re invested into the local community.

What does this mean for patients?

You will start to see our logo around the surgeries and on certain documents as we develop our collaborative working.

You will be able to benefit through shared learning of best practice and improved services.

We are working closely with our partners throughout the local health service, social services and voluntary sector to develop new pathways and improve existing services to enable seamless, efficient and coordinated care.

We are very keen to develop a Oldham West PCN patients and partner participation group to work with us on the journey.

Practice Charter

Werneth Medical Practice’s mission is to provide the highest standard of patient care though a framework we call the 4 H’s:

In return, we will demand that patients treat us with kindness, respect and patience and allow us to do our job free of abuse and undue pressure.

Our team includes Two GPs, One Practice Nurse, one Phlebotomist as well as our Practice Manager and Reception staff.  We also now have allied staff via our Primary care network such as Social prescribers, Pharmacists and First contact physios.  Dr Bilal Butt is the CQC registered manager and we run our services at the Werneth Primary Care Centre on Featherstall Road South.

We offer a full General Practice service and run specialist clinics for children, those with chronic diseases such as Diabetes and Asthma and women’s health.  We aim to involve you in the decisions made about your health and act as advocates for your best interests.

At Werneth Medical Practice, we aim to treat all our patients promptly, courteously and in complete confidence. We feel it is important you know who you are speaking to, so our practice staff identify themselves on the telephone.

National Data Opt-Out

NHS Digital has been directed to collect, process, analyse and disseminate general practice data to support the purposes set out in the GP Data for Planning and Research Direction 2021.  This is a national initiative and from July 1st, 2021, patient data will be extracted from  GP systems on a daily basis; data will be encrypted in transit and pseudonymised.

As this is a statutory obligation, practices are legally obliged to share this data, unless patients have opted out of having their data shared.  If you as patient want to opt out of your data being shared, apart from information used for your direct care,  you have the option of registering a Type 1 Opt-out or a National Data Opt-out, or both.

Chaperones

You have the right to request and clinicians should offer a chaperone for all personal/intimate examinations.  Our Chaperones are specially trained and will witness the examination for the piece of mind of all concerned.  Please request at the point of need.

Patient with Particular Needs

Our surgery is accessible to patients using a wheelchair.

We also have parking space outside the practice which can be used.

Please inform us in advance if you require an interpreter so one can be booked.

We may also use a telephonic interpreter using ‘Language line’

Violent & Abusive Patients

We aim to treat our patients courteously at all times and expect our patients to treat our staff in a similarly respectful way.

We take seriously any threatening, abusive or violent behaviour against any of our staff or patients. If a patient is violent or abusive, they will be warned to stop their behaviour.

If they persist, we may exercise our right to take action to have them removed, immediately if necessary, from our list of patients.

Patient Confidentiality

We respect your right to privacy and keep all your health information confidential and secure.

It is important that the NHS keeps accurate and up-to-date records about your health and treatment so that those treating you can give you the best possible advice and care.

This information is only available to those involved in your care and you should never be asked for personal medical information by anyone not involved in your care.

You have a right to know what information we hold about you. If you would like to see your records, please call our Practice Manager.

Complaints

If you are dissatisfied with the service we provide, please read our complaints leaflet below which explains how to make a complaint.  We WANT to know if any part of our service doesn’t meet our high standards.  Please note, we cannot investigate any complaints about other service providers or regarding waiting times for referrals.  If your complaint refers to these, please contact the service provider directly, e.g if it concerns hospital care, contact PALS in the hospital concerned.

If you would like to make a complaint on behalf of a patient please complete the following consent form and return to the practice.

We are extremely busy at the moment.  Although we will endeavour to keep within the time scales mentioned in our policy, there may be a short delay while the matter is investigated.

You have the right to be unhappy if you feel our service doesn’t meet your reasonable expectations.  However your unhappiness should be proportionate, measured and specific.

You do not have the right to abuse, insult, threaten, be violent or intimidating to any of our staff.  Either in person, on the telephone or online.  We have a zero tolerance to this behaviour and will remove these individuals from our patient list.  We will also involve the Police where needed and fully engage with any legal process.

How we store it

The Practice will retain your personal data as long as it is required for the purpose for which the data is collected. Any data submitted via online forms are also retained on the web hosting server for a period of time specified by the practice, default period is 30 days. Backups of this data is stored by SurgeryWeb for a maximum period of 14 days at which time this data is auto-deleted.

The data centre is UK based and all data is encrypted when stored and in transit such as when you submit a form to us. This website is https-secured which means communication between the users web browser and the server hosting this website is encrypted and cannot be intercepted en-route, this can be verified by the padlock icon in the address bar.

Access to your information

In accordance with the Data Protection Act 2018, you have the right to request a copy of the information that we hold about you, if you would like a copy of some or all of your personal information, please contact the practice.

Erasure of Data (right to be forgotten)

You have the right to have your data erased if the personal data is no longer required for the purpose it was originally collected for, if we are processing the personal data for direct marketing purposes and you object to that processing, or if you believe we are processing your personal data unlawfully.
